• You understand that e-mail is not a secure method to communicate medical and personal issues. There is a risk that a 3rd party could obtain information in the transmission of an e-mail.

• Contact your treating physician with any medical question first and contact 911 in the event of a medical emergency.

• If you e-mail me, you are granting me permission to read the information provided.

• I will only use the e-mail address provide by you, unless you notify of any changes. Any first email will be subject to verification (usually a phone call) because  I cannot assume the sender of an e-mail is the person he or she claims to be.

• Minimize the amount of information disclosed in en e-mail communication.
